We appreciate your business. As a new customer, let me familiarize
you with our credit terms. Upon credit approval, our standard terms are net 30 days of the invoice date. A service charge of 1.5% monthly - 18% annually is charged on past due accounts.

If you have not done so, please take the time to fill out and return the credit application found on these pages. An incomplete form may result in a delay when processing your order and, thus, may be an inconvenience to you.

Feel free to send standard forms that list your suppliers and bank references along with this application. However, be sure that the Customer Information Section is filled out entirely and it is signed in the authorized signature area on the lower section of the form.

The completed application, Owner Information and Sales Tax Exemption Certificate (if applicable) can be returned via fax at 253-234-3039 or by mail. Please mail your completed forms to the following address.
